As Big Data takes hold in healthcare circles, and systems are increasingly centralized (whether in cloud or not), errors like this one should be anticipated.

The threshold for tolerable error may be low in medicine, but it probably needs be even lower.

Mark,

I'd agree with you on the need to be far less in errors in certain cases.

If I understand the graphic you supplied appears to be a record from 2005. If true, the error rate should be zero. Records are a completely different business model and critical to forensics and audit ability.

Especially, in the example you shared.
